Forms and Fees
Prior to our first session together…
I will ask you to complete/sign the forms below. These forms may be downloaded from this site, or, if you prefer and time allows, I will email or mail them to you. Either way, these documents are intended to give you a greater idea of the nature of my therapeutic work and the counseling process, and to let me know what some of the general areas of our work together will be.
Please bring these documents to our initial session and, in the meantime, do not hesitate to call or email should you have a questions. A few helpful hints/notes are listed below the attached forms.
Please bring a copy of your insurance card - front and back.
For couples seeking counseling, each individual should complete a full packet of their own (each of the below forms) as information and symptoms are uniquely individual.
Please note that for children/adolescent clients under 18 years of age receiving individual therapy, a parent or guardian will need to complete the consent and release of information forms.
Fees & Payment
I accept Blue Cross/Blue Shield Federal, PPO plans, and some HMO plans, as well Cigna Behavioral Health.
For all other insurance plans, I am considered an "out of network provider" and depending on your plan, you may be eligible for possible reimbursement for a portion of the cost of therapy fees. We can explore this together if you'd like. All fees for therapeutic services are due at the time of each session and may be paid either in cash, by personal check, or through your Visa or Mastercard.
Please be aware that reimbursement from your health plan is not guaranteed and requires a diagnostic statement that will remain on your permanent insurance record. Whether or not you choose to submit paperwork to your health insurance plan for possible reimbursement, payment of all fees for therapeutic services rendered is due at the time of your session unless other financial arrangements have been made in advance.
